All Athlete’s must complete the NEISD Electronic Athletic Participation Form (Click here).  Select High School. Click on Athletic Participation Packet (High School). Fill in all required fields and click Submit. A copy of the completed form will be e-mailed to you.  Under Printable Forms, download the Medical History and Physical Form to take to your physician. Once complete, return the form to your Coach or Athletic Trainer.
